Forms
1. Please fill out the following forms if you are a new client.
2. If you can please bring them with you to your "initial" appointment that would be ideal.
3. If you cannot print out the forms, please ask us to do so for you, but come 30 minutes earlier for your appointment so you have time to fill them out.
